1. EFFECTIVE DATE

Unless otherwise provided herein, the Membership Agreement shall commence on the date specified in the electronic mail (email) received, once membership has been successfully registered and shall be effective for a term of one (1) year. Membership may be renewed on a yearly basis subject to mutually agreed upon terms and conditions.  

2. TERMINATION OF MEMBERSHIP AGREEMENT

Within fifteen (15) days from receipt of this Agreement, the Member may cause the cancellation or revocation of the Membership Agreement by answering the Refund and Cancellation Google Form. For the avoidance of doubt, any PRIMA Membership, whether new, higher-tier, or existing, shall not be refundable, whether in full or in part, beyond the fifteen (15)-day cancellation period. Failure to cancel or revoke this Agreement within the period set shall be understood as an acceptance of all the terms and conditions provided hereunder. Any availment within the 15-day period shall also mean acceptance of this Agreement.

MaxiHealth shall have the right to immediately terminate the Membership Agreement in the event that:

1. Any material representation or warranty made by Member is false or untrue when made; or if Member commits any act with the intent to defraud MaxiHealth;  and
2. If Member is in material breach of the Agreement and has failed to cure such breach within thirty (30) days after its receipt of written notice from MaxiHealth

All Medical Services and coverage under this Agreement shall terminate on the termination date, without prejudice to any claim for covered Medical Services rendered to a Member prior to the termination date.

3. REFUND

There will be no refund of Membership Fee after fifteen (15) days from receipt of the Membership Agreement.

A Member who engages in any fraudulent, unauthorized, improper, misleading, or otherwise prejudicial act or conduct in connection with the enrollment, administration, or availment of membership benefits or services, whether committed intentionally or otherwise, may be deemed ineligible for any refund, payment, reimbursement, or other benefit otherwise available under the Membership Agreement, without prejudice to MaxiHealth’s exercise of any other rights or remedies available under this Agreement or applicable law.
